An International Wake-Up Call

Got another real life case I’ll share with you. A guy I’d guess to be in his mid 40s tells me he got labs a year and a half ago that showed a total testosterone of 280. Didn’t surprise him. He was feeling it. Over the next year he went back and forth with his doctor who tested him three more times and got levels in the low 300s. Just enough to slip into that 300-900 brackets his doctor considered the no treatment range. He was told it’s just not low enough to get help. But, he told the doctor, it was sure low enough to not feel good. Then he said, and I love this, “If you were used to having $1 - $1,000 in your bank account, and I said you now have $3, would you feel comfortable?” Brilliant! And it turns out this is just the kind of common sense that experts have known to be true for a good decade now. I don’t know why we are still struggling to understand this. In 2015, a respected international expert panel made up of a broad range of medical specialties debated and unanimously approved 9 resolutions that should put this to rest. One of those resolutions says there is no T concentration threshold that reliably distinguishes those who will respond to treatment from those who will not. They understood and the data supports that blood levels are just one metric used to determine if someone could benefit from testosterone supplementation, and not a very good one at that. I will leave you to read the other 8 resolutions, but they are eye-opening and the myths about testosterone treatment that they dispel will be surprising to your doctor.
